_______________________, who are applying for a simple building
permit. I declare that I have read and understand the regulations contained within the current Maple Ridge Building
Bylaw.
a. I acknowledge that the activities undertaken by or on behalf of the City of Maple Ridge pursuant to this Bylaw
are for the sole purpose of providing a limited spot check for health, safety and the protection of persons and
property. Furthermore, I confirm that I have read and understand Section 3 “Purpose of Bylaw” in its entirety.
b. I acknowledge and confirm that I have read and understand Section 4 “Permit Conditions” in its entirety.
c. I acknowledge and confirm that I have read and understand Section 18 “Disclaimer of Warranty or
Representation” in its entirety.
d. I acknowledge and confirm that I have read and understand Section 20 “Responsibilities of the Owner” in its
entirety.
e. I acknowledge and confirm that I have read and understand Section 22 “Inspections” in its entirety.
f. As the City of Maple Ridge may require that Letters of Assurance be provided by a Registered Professional
pursuant to the District of Maple Ridge Building Bylaw and the Local Government Act, I confirm that the
following list of Registered Professionals are reviewing plans, drawings, specifications and supporting
documents submitted with the building permit application.

g. I confirm that I have relied only on the said Registered Professional for the adequacy of plans, drawings,
specifications and supporting documents submitted with this permit application.
h. I understand that I should seek independent legal advice in respect of the responsibilities I am assuming
upon the granting of a building permit by the City of Maple Ridge, pursuant to the application and in respect
of the execution of this acknowledgment.
