STAFF BENEFITS HOURLY SALARY RANGE (Effective April 1, 2012) $ $ $ $ VACATION SCHEDULE Calendar Year Days Vacation 1 1.25 days per month (pro-rated as per Article 16.01 of Collective Agreement) 2 to 7 15 8 to 12 20 13 to 19 25 20 + 30 MEDICAL SERVICES PLAN – commences 1st day of calendar month following 3 months of continuous employment • Basic Provincial Medical and Surgical coverage • 75% of the premium is paid by the employer DENTAL PLAN – commences 1st day of calendar month following 3 months of continuous employment • Covers 100% of plan A - Routine Treatment - which includes: preventative services, x-rays, extractions, fillings, root canals, and treatment of gums. • Covers 70% of plan B - Major Treatment - which includes: crowns and bridges, onlays and inlays, and partial or complete dentures. • Covers 50% of Plan C - Orthodontics – lifetime max. of $3000 per dependent • Combined Plan A & B expenses are covered to a maximum of $2000 per dependent each calendar year • Eligible dependents are covered to age 21 (or to age 25 if they are full-time students). • 75% of the premium is paid by the employer LIFE INSURANCE – commences 1st day of calendar month following 3 months of continuous employment • Coverage is 2X annual salary to a maximum of $400,000. • 75% of the premium is paid by the employer Please initial _________ EXTENDED HEALTH BENEFITS – commences 1st day of calendar month following 3 months of continuous employment • Includes: vision care to $400 every two years; hearing aids to $400 every two years; out-of-country care expenses; prescription coverage; and the additional limited coverage for the services of a: psychologist, speech therapist, chiropractor, naturopath, optometrist, physiotherapist, and podiatrist. • Expenses are subject to a $100 annual deductible and are reimbursed at 80% of the first $1000, and 100% thereafter. • Eligible dependents are covered to age 21 (or to age 25 if they are full-time students). • 80% of the premium is paid by the employer • Direct payment card ACCIDENTAL DEATH AND DISMEMBERMENT – commences first 1st day of calendar month following 3 months of continuous employment • Coverage for loss of life as the result of an accident in the amount of the life insurance. Defined coverage for other specific losses/dismemberment resulting from an accident. • 75% of the premium is paid by the employer LONG TERM DISABILITY PLAN – commences first 1st day of calendar month following 3 months of continuous employment  Coverage to replace income lost because of a lengthy disability due to illness or injury.  After a six month waiting period, benefit entitlement is 70% of the first $3000 of monthly earnings plus, 50% of the remainder to a maximum of $10,000 per month  100% of the premium is paid by the employee SICK LEAVE • Earned at the rate of 1 1/2 days for each month of service. May be accumulated to a maximum of 250 days. PENSION PLAN–commences after three months. If already enrolled commences immediately . • Provisions are granted in accordance with the Public Sector Pension Plans Act. • A defined benefit pension for which the employer pays approximately 10.38% of regular earnings and the employee pays approximately 8.5% in contributions to the years maximum pensionable earnings then 10.00% to the end of the year. • Pension amount is 2% for each year of pensionable service, up to 70% (including CPP benefits) of the average annual salary earned in the highest five earning years before retirement. SERVICE SEVERANCE PAY • Employees who retire (as defined by the Superannuation Pension Act) with a minimum of ten (10) years of service with the Employer, shall be paid twenty (20) days pay upon retirement.
